Generally referrals are made by a professional involved in meeting the care needs of the individual. Typically this is a Social Worker. We do also accept self-funders however, and if you are the person wanting support or a family member of the person wanting support, and you will be funding the placement, please contact us.

Generally referrals are made by a professional involved in meeting the support needs of the individual. Typically this is a Social Worker for both Adult Care funded support and Supporting People funded support, though we do accept referrals from other support providers for Supporting People funded support as well. We do also accept self-funders and if you are the person wanting support or a family member of the person wanting support, and you will be providing the funding, please contact us.

Eligibility criteria:

  • Sp funded support
  • Support funded otherwise (either Adult Care or self-funding)

If the referral made is eligible, we will arrange a meeting with the individual preferably at their current residence to conduct a needs and risk assessment. A second meeting can take place if required, and the individual is invited to the office base to help them make an informed decision about whether they want our service to support them.
